UHSAA Pass Request Form

Picture ID must be shown when pass is used. THE ALLOTMENT OF PASSES FOR 3A SCHOOLS IS 55 (65 if your school has 9th grade in the building). This number will include 4 complimentary passes. The additional passes (7 of which may be discretionary) can then be ordered at a cost of $35.00 each, to a total of up to 55 (65) passes. Qualifications for receiving passes will still remain the same, i.e. passes will be issued for principals, assistant principals, athletic directors, current athletic coaches, one drill team advisor, one cheerleader advisor and team physicians. The discretionary passes that are ordered must be for high school staff members affiliated with the activities programs (band director, forensics advisor, etc.).

PLEASE LIST ONLY THOSE PEOPLE WHO QUALIFY. ALL POSITIONS MUST BE INDICATED ON THE FORM OR PASSES WILL NOT BE SENT.

Exchanging a pass for a different name will only be allowed until October 1. After that time, a new pass will need to be purchased and will count toward your allotment.

Please do not order passes for Silver Pass holders. You can request the silver pass application form from Jennifer Thomas at jthomas@uhsaa.org.

Press submit when done and an invoice will be sent to your school to collect the total amount owed.
